Medical inflation (or medical trend) is the yearly rise in health insurance claims cost per person, driven by treatment prices, new technology and higher use. In Thailand it runs many times above general inflation, and it is the main reason health premiums rise every year.

For insurers, the problem is that the traditional levers (premium rises, benefit cuts and co-payment) all pass cost on to customers and risk losing them at renewal. The lever getting more attention is lowering cost per case without cutting cover, by guiding members to suitable, good-value care before costs are incurred.

Medical inflation in Thailand: the latest figures

SourceYearForecast for Thailand
Aon Global Medical Trend Rates (October 2026)202614.8%
Aon Global Medical Trend Rates (October 2026)202711.7% (10.7% net of general inflation)
WTW Global Medical Trends Survey202610.8%
Mercer Marsh Benefits Health Trends202614.6%

Each source collects data differently, so the numbers differ, but all point the same way: costs are still rising by double digits a year, while ttb analytics estimates Thai household income grows only about 3% a year. That gap makes higher premiums harder for customers to accept every year.

Mercer Marsh Benefits reports that most insurers in Asia see higher incidence of conditions and changes in treatment as the main drivers, not price alone. Cost can therefore be managed both through price per case and by getting members to the right care from the start.

The cost-containment levers insurers use

LeverWhat it doesSide effect
Premium increasesCovers higher costsCustomers compare and switch at renewal
Co-payment or deductiblesReduces unnecessary claimsSome members delay necessary care
Claims review and fraud detectionCuts incorrect paymentsWorks after the cost is incurred; strains hospital relationships if too aggressive
Provider networks and agreed pricesLowers price per caseOnly works if members actually use the network
Care navigationLowers cost before it is incurred, without cutting coverNeeds a team, price data and members’ trust

The first three act after the member has decided. The last two act before the decision, which is where cost can still change the most. Thailand’s standard health insurance has also included a co-payment condition at renewal since 20 March 2025.

How to steer members to high-value care

Prices for the same procedure vary widely between private hospitals in the same area, because of location, target customers and cost structure, while quality for routine procedures is often similar. Before the member has decided, good advice helps them choose the better-value option.

  1. Start with planned procedures such as elective surgery, endoscopy and high-cost imaging: there is time to advise and the cost per case is high
  2. Group providers by value: quality, outcomes, expertise and price per case, then name the recommended options in each area
  3. Use agreed package prices, which remove uncertainty about the final bill for insurer and member alike
  4. Reach members before the appointment, for example at pre-authorisation or when they ask about cover, and offer options with reasons
  5. Use incentives, not compulsion: no upfront payment, help with booking, or lower co-payment for the recommended option where the policy allows
  6. Choose the right setting: some treatment can be outpatient or day surgery where medically appropriate, at lower cost than an inpatient stay
  7. Offer second opinions before major surgery to confirm it is appropriate

For example, a procedure averaging THB 120,000 at higher-priced hospitals may have an option of suitable quality in the same area averaging THB 85,000. With 400 such cases a year and 30% of members choosing that option, treatment cost falls by about THB 4.2 million a year from one procedure. These are made-up figures; try your own portfolio below.

Earning members’ trust

  • Explain the reasons clearly: the doctor’s expertise in this procedure, an all-inclusive package price, no upfront payment
  • Members can always choose, and cover is not reduced if they stay with their usual hospital under the policy terms
  • Help throughout: booking, paperwork, the day of treatment and follow-up
  • Never speak badly of any hospital: say why the recommended option is good, rather than that another is too expensive

Measuring cost containment

How HDmall and HDcare help

HDmall lists more than 30,000 health services from over 2,500 hospitals, clinics and labs across Thailand, with prices shown before booking. HDcare’s care team guides members to suitable providers before costs are incurred, and most users save 15–20% against market prices. Frequently asked questions

What is medical inflation in Thailand in 2026?

It depends on the source. Aon forecasts 14.8% for 2026 and 11.7% for 2027; WTW forecasts 10.8% and Mercer Marsh Benefits 14.6% for 2026. All are many times above general inflation.

Why do medical costs rise faster than general inflation?

Several factors at once: more illness as people age and lifestyles change, newer and more expensive technology and medicines, higher use of services, and private hospital pricing. Mercer Marsh Benefits reports that most insurers in Asia see higher incidence of conditions as the main driver.

What is care navigation?

Care navigation guides members to suitable treatment, doctors and facilities before costs are incurred, unlike claims management, which deals with care after the fact. It helps members get appropriate care and helps insurers control cost per case.

Does steering members to lower-priced hospitals reduce quality?

It should not, if options are chosen on value: quality, outcomes, expertise and price together, never price alone, with outcomes measured alongside cost. Members must keep the choice.

How can insurers reduce over-claiming without hurting customers?

Combine levers: detect unusual claims with data, agree package prices with network hospitals, and guide members to appropriate care before booking. That removes unnecessary spend without cutting cover or burdening members who claim legitimately.

Which procedures should navigation start with?

Planned procedures with enough volume and wide price variation between providers, such as elective surgery, endoscopy and high-cost imaging, because there is time to advise and the saving per case is high.
